Due to COVID-19, and the impact on the nursing care industry, a survey web form was needed to screen employees and visitors. Employees are required to answer health questions and have their temperature taken at the beginning and middle of their shifts. Visitors, vendors, and contractors were also required to answer health questions prior to visiting the facility. The employees performing the screenings needed a simple web application that could work on desktop or on tablet devices. The responses needed to be persisted in a database for monitoring compliance. The screeners needed the ability to lookup employee names.
The solution leveraged a responsive web application to deliver three survey forms with a specific set of questions for both desktop and tablet. Two forms were provided for employees for the beginning and mid-shift survey questions. The temperature pad reduced touches by adding the decimal in for the user. The employee name lookup’s leveraged real time autocomplete lookup’s the user could select from in a drop down list. The survey responses were persisted to a SQL Server database repository where they could be reported on for compliance.
The solution leveraged ASP .Net MVC with Razor, Entity Framework, SQL Server, JQuery, and Material Design Lite for the responsive web framework.
